MHO Proprietary Tools


Psychosocial Assessment: Our most frequently customized tool is also an excellent example of how clinicians integrate outcomes measurement with their daily clinical routine, capturing information known to impact treatment planning, provision and outcome.

Medical Record Information Form: Captures variables that can be obtained directly from the medical record. It provides insight about patient status at discharge and also gives information about variables that may have predictive value in relation to outcome.

Medication Usage Questionnaire: Captures psychotropic medications before, during and following treatment, allowing for benchmarking of the types and numbers of medications prescribed and impact on treatment outcome.

Patient Satisfaction Form: Assesses the patient's experience with the treatment program, providing an opportunity for the patient to describe different phases of their experience. Some of the questions used were derived from the Patient Judgments of Hospital Quality questionnaire.

Follow-Up Questionnaire: Completed during a telephonic interview conducted by MHO staff post-discharge. Many of the items are comparable to items obtained at the time of admission or discharge, allowing for direct comparisons of the variables measured at different points in time.


Custom Tools


In addition to utilizing the standard CQI+ Instruments, MHO clients may create their own tools or utilize any public domain or proprietary instruments for which permission is obtained.
